In consideration of being allowed to participate in any way in the United Filipino Basketball of Hamilton, basketball program, related events and activities, I, the undersigned, acknowledge, appreciate, and agree that:

1. Risk of Injury: The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death. While particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist.

2. Health Condition: I am in good health and have no physical condition that would prevent me from participating in this sport. I have adequate health insurance necessary to provide for and pay any medical costs that may directly or indirectly result from my participation. I agree to bear any costs incurred as a result of my participation.

3. Release: I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releasees or others, and assume full responsibility for my participation.

4. Waiver: I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, hereby release and hold harmless the United Filipino Basketball of Hamilton, its officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event ("Releasees"), from any and all liability for injury, disability, death, loss or damage to personal property associated with my presence or participation, whether arising from the negligence of the releasees or otherwise, to the fullest extent permitted by law.

5. Consent to Medical Treatment: I consent to have medical treatment that may be deemed advisable in the event of injury, accident, and/or illness during this event. I release all Releasees from any claim whatsoever which may arise as a result of such first aid, treatment, or service.

6. Acknowledgment: I have read this release of liability and assumption of risk agreement, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing the agreement freely and voluntarily, and intend by my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
